Methodology and assumptions

Weighting · Questions are weighted by their severity as a breach (e.g. failing to obtain a 408(b)(2) disclosure is heavier than missing a Plan Governance Committee). Total weight sums to 100.

Polarity · Most questions are positive ("yes" is good). The laser-provisions question is inverted because lasers indicate stop-loss vulnerability.

"Unsure" · Treated as a partial breach. We award 35% of the question's weight, on the theory that documented uncertainty is materially less bad than documented absence.

Action plan · The top three remediation items are ranked by the point gap between your answer and the best-case answer.
